Canthal tilt describes the angle of the axis running from the medial canthus, the inner corner of the eye, to the lateral canthus, the outer corner. Measured against a horizontal reference line, a positive angle means the outer corner sits higher than the inner corner, and a negative angle means it sits lower.

The measurement comes from periocular anthropometry, where inner and outer canthal distances are standard landmarks. Most published adult samples cluster slightly positive, but the spread across individuals and across ethnic groups is wide enough that no single number is a target.
